Currently, we work exclusively with Lifesong for Orphans to place children with adoptive families. We ask that you complete the application provided below and submit it directly to Lifesong. If the application meets Lifesong’s qualification guidelines, it will then undergo our screening process. You will be notified directly by Lifesong if you qualify for adoption assistance.
Forever Adoption grant requirements:
1. You may submit this application only after your home study is complete.
2. This application is for domestic and international adoptions.
3. Applications from all qualifying U.S. citizens will be reviewed.
4. Lifesong for Orphans must receive the completed application before the placement of a child in your home. This allows both Lifesong and Forever Adoption to comply with IRS guidelines and ECFA best practice.
5. You may only submit one Forever Adoption grant application per adoption to Lifesong.
